After a career in healthcare, I found my true calling in building meaningful relationships to help people navigate some of life’s most challenging transitions. I am a Certified End-of-Life Doula, Elder Care Doula, and Doula Care Consultant. I'm also a proud member of the International Doulagivers Membership Association and the National End-of-Life Doula Association. I currently serve as a hospice volunteer doula and was honored to be the 2024 recipient of the Donna West Memorial Award (North Region). I am devoted to guiding individuals and families through life’s most tender transitions. With compassion and deep listening, I support those facing life-limiting illness in finding peace, closure, and dignity. I also help with Advance Care Planning to ensure your wishes are known, respected, and honored. It would be my privilege to walk alongside you and your loved ones during life’s most meaningful moments.
Life’s transitions can feel overwhelming—you don’t have to face them alone. Compassionate Transitions offers non-medical, whole-person support for individuals and families navigating aging, advance care planning, and end-of-life, honoring each person with dignity, compassion, and care.

Over the summer, we became aware that there is such a thing as an end of life doula. We contacted Nancy Macke at Compassionate Transitions to learn more about it. We instantly felt comfortable with her and have been working with her since. It has been one of the best decisions we have made. She's been helping with all aspects of navigating this disease and what comes with it. From practical to personal, and everything in between. She's been part legal researcher, medical interpreter, communications advisor, recording secretary, historian, archivist, therapist, friend and so much more.

Nancy's calm, wise manner makes difficult topics easier to explore. It all was valuable from the info Nancy provided to the conversations/questions from the participants." "Everyone needs to plan for their own death, and Nancy coaxed us to get going on our wishes, then share them with MD and our family." "This workshop will provide you with the valuable info to protect you from unnecessary end-of-life procedures you do not want. Get your ducks in a row." "Absolutely informative sessions to learn, talk, and get your ducks in a row for leaving this world how you desire, gracefully.

We are so grateful for you. Thank you for guiding us through this journey. We will never forget your grace, kindness, and steady hand.
What types of services do you provide?
I offer comprehensive non-medical end-of-life doula services including compassionate end-of-life support, advance care planning, and elder care doula support for individuals and families.
What should clients know about your pricing(e.g., discounts, fees, packages)?
Pricing depends on the services provided. Discounted rates are available for prepaid blocks of 10 or more hours of end-of-life doula support. Please call to discuss pricing and service details.
What is your typical process for working with a new client?
All new clients are offered a complimentary 30-minute consultation. Services are thoughtfully tailored to each individual and family, recognizing that end-of-life care is never one-size-fits-all. For advance care planning and elder care doula services, an introductory call helps us mutually determine needs and next steps.
What are some of the ways you have supported someone at end-of-life?
Planning for End of Life Support with clarifying and documenting wishes, advance care planning, legacy projects (letters, memory books, remembrance gifts), and facilitating family conversations. Through the End-of-Life Process Advocacy for wishes and needs; coordination with hospice, medical providers, and care facilities; assistance with hospice selection and discharge planning; vigil sitting and compassionate presence as life draws to a close. After Death Remaining with the client until the funeral home arrives; assistance with funeral and body disposition planning; liaison with funeral homes and providers; help with obituary and eulogy writing; and continued support for loved ones, including education, emotional support, and guidance with post-death administrative needs.
What advice would you give someone looking to hire an end-of-life doula?
I encourage individuals and families to speak with several end-of-life doulas to understand each doula’s approach, philosophy, and services. This is a deeply personal time, and the relationship should feel comfortable, supportive, and like a good fit for you and your family.
What questions should clients think through before talking to doulas?
Some things to consider before talking with a doula might include: What has prompted you to reach out, and is there an immediate need or are you planning ahead? What does a 'good death' look like to you? Are there conversations you've been putting off, and what kind of support are you hoping for?
Do you offer any other services beyond end-of-life doula support (e.g. advanced care planning, paralegal, legacy services, chaplaincy services)?
Beyond end-of-life doula support, I offer comprehensive advance care planning and legacy services. In addition to the completion of your Living Will and designation of Healthcare Power of Attorney, I help clients create a personalized Living Well end-of-life plan, supporting smoother, more intentional transitions through aging and end-of-life.
Do you work as part of a collaborative or independently?
While I practice independently, I regularly collaborate with fellow end-of-life doulas and maintain a broad network of allied professionals — ensuring clients have access to the full spectrum of support they may need.
Are you willing to work remotely?
My preference is to work with clients in person, as the nature of this work is deeply personal. However, I am able to support Advance Care Planning discussions and document completion virtually for those who are unable to meet in person.
