Health Care Proxy

A health care proxy is a person who can make medical decisions for someone else if they are unable to make decisions for themselves.

A health care proxy has the legal authority to choose medical treatments, consent to medical procedures, and decide on end-of-life care.

They are responsible for making decisions that are match the wishes of the individual they represent as outlined in a Living Will or Advance Directive.

Also known as a Health Care Agent or Medical Power of Attorney.
